Final Forms

We are very excited to announce that Columbus Grove Local Schools has partnered with FinalForms, an online forms and data management service that will be replacing paper forms. FinalForms allows you to complete and sign enrollment, back-to-school, and athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. 

IMPORTANT: Starting Monday, November 18th, emails will be going out to all parents that will help you to create your account and begin entering information. If you don't see an email by Tuesday, please check your spam folder, and if you can't find it, please contact Mr. Keehn and he can make sure we have the correct address on file. (NOTE: It is not necessary for parents of seniors who are the only child in school to create an account or fill forms out.)

Though we collected information on paper at the beginning of this year, to get things into the new system we ask that you log in to review your data at any time to verify it is current. The average time to complete the forms is around 20 minutes. We appreciate the time you take to enter the information! Going forward you will be required to sign your forms once per year and after any update. The below instructions outline the process to get started in FinalForms.
