Obtaining a Work Permit

To avoid delays in the work permit process during the summer months, please follow these steps…
1. Print out a pre application form that can be found here.
- The top portion needs to be filled out and signed by a parent. 
- If a physical is not on file at the school within the year, please print the physicians form found here and have your family physician fill it out.
- Have the employer fill out the “Pledge of Employer” portion.
2. Contact the school to arrange a time to process the form. If before June 14th, please call Cari Fuerst at 419-659-2639. If after June 14th, please email Kristal Pingle.
3. Bring the completed form to either Cari or Kristal (during the arranged time) to have the info entered into the Ohio.gov website. Both the student and the secretary will have to sign the completed form to be taken back to the employer.

Substitute Bus Drivers Wanted

Columbus Grove Local School is accepting applications for substitute bus drivers.  This position has the potential of becoming a full-time bus driver in the future, if the applicant is interested.  Substitute bus drivers are contacted to drive extracurricular and field trips and occasionally drive a regular morning or afternoon route.  Certification requires the completion of attending a pre-service class, a written and driving test, CDL license, a medical physical exam, drug test, BMV check of driving record, and a background check.  Cost for training will be reimbursed by the district for any individuals who serve as a substitute bus driver for Columbus Grove Local Schools. Student Responsibilities

Your device is an important learning tool and is to be used for educational purposes only. In order to take your device home each day, you must be willing to accept the follow responsibilities:

• When using the device at home, at school, and anywhere else I may take it, I will follow the policies of Columbus Grove Local Schools, especially the Student Code of Conduct and the Acceptable Use and Internet Safety Policy (Policy 7540.03), and abide by all local, state, and federal laws.
• I will treat the device with care by not dropping it, getting it wet, leaving it outdoors, or using it with food or drink nearby, and I will keep my device in my school-issued protective case at all times.
• I will not lend the device to anyone, not even my friends or siblings; it will stay in my possession at all times.
• I will not load any software onto the device that is deemed inappropriate
• I will not remove programs or files from the device.
• I will not give personal information when using the device.
• I will bring the device to school every day.
• I will not use my device to access personal email accounts such as Gmail or Yahoo, not pertaining to classroom activities.
• I agree that email (or any other computer communication) should be used only for appropriate, legitimate, and responsible communication.
• I will keep all accounts and passwords assigned to me secure, and will not share these with any other students.
• I will not attempt to repair the device.
• I will return the device when requested and upon my withdrawal from Columbus Grove Schools.
